Indian firm gets Tamiflu licence

Swiss firm Roche has licensed an Indian company to make a generic version of its anti-flu treatment Tamiflu for India and other developing countries.

The drug is widely seen as the most promising treatment for combating any future pandemic of bird flu in humans.

The previously little-known Indian pharmaceutical firm Hetero Drugs has been granted the licence.

The decision could make it easier and cheaper for poorer nations to acquire stocks of the drug.

Reliable version

The Swiss drugs giant developed, and has patent protection for, a product that is in great demand from governments around the world.

They all want to build up stocks of Tamiflu - known generically as oseltamivir - to cope with the anticipated spread of bird flu to humans.

This appears to be only the second such manufacturing licence Roche has awarded.

The first one went to a Chinese firm, the Shanghai Pharmaceuticals Group.

It is a sign that Roche is responding to intense pressure from poor countries.

They have been worried that Roche on its own cannot make enough of the drug to meet demand - and that the prices it charges Western governments are too high for developing nations.

Roche meanwhile has claimed that most other drugs firms do not have the skills to make Tamiflu, a year-long process involving fermentation of e-coli bacteria.

The company says Hetero Drugs is among the first to demonstrate that it is technically capable of making a reliable generic version of Tamiflu.

In Asia, H5N1 had already evolved resistance to another class of antivirals, including amantadine, by 2003. These resistant strains are just as deadly and contagious as non-resistant strains. "The Chinese have been incorporating amantadine in their chicken feed, so we have lost that as a treatment," says Robert Webster of St Jude Children's Research Hospital in Memphis, Tennessee. This also suggests that Chinese farmers have been fighting bird flu outbreaks since before 2003, although China officially reported its first outbreaks only last year.

Tamiflu can save lives if it is given early, no more than two days after symptoms first appear. But last month the WHO reported that a patient in Vietnam had a strain of H5N1 resistant to Tamiflu. So could the drug become useless before the pandemic even begins?

Luckily the resistant viruses may be poor at spreading, according to Fred Hayden of the University of Virginia, a leading expert on antiviral therapy. The mutation that made the Vietnam virus drug resistant also occurs in a normal human flu strain, making the virus a hundred times less contagious. In Japan, which uses Tamiflu for ordinary flu epidemics, the mutation appears in 16 per cent of treated children, yet such viruses almost never go on to infect others.

It is probable that the same will be true of any drug-resistant strains of H5N1. This assumption needs to be tested as soon as possible, Hayden says.

But even if Tamiflu remains effective in most cases, it might not be enough to stop a pandemic. The real difficulty with the WHO's antiviral plan, Hayden says, will be finding and treating all the cases and contacts in time. "But that doesn't mean we shouldn't try," he adds.

Ira Longini of Emory University in Atlanta, Georgia, says much depends on how fast the virus spreads. If each infected person passes the virus to fewer than two other people on average, then isolating and treating all cases and their contacts with antivirals could slow or even stop an epidemic, he calculates.

But health workers would not be able to keep up with the virus if sick people infect between two and three others, as happened in the 1918 flu pandemic. Drug stockpiles would still help save lives, Longini says, but would not halt the outbreak.

The best chance of the antiviral strategy succeeding will be in the early stages, when the virus might still spread slowly. The trouble, however, is that most stockpiles of Tamiflu are being acquired by rich countries in Europe and North America, not poor countries such as Vietnam, where any H5N1 pandemic is most likely to start.

What's more, Tamiflu is in short supply. Seventeen countries have ordered stockpiles of the drug from the Swiss company Roche, which holds the patent, and 10 more are said to be discussing purchases. The UK's order for 14.6 million five-day courses of treatment will take two years to fulfil, for instance. The drug is made from a plant in limited supply, and Roche is still trying to develop methods for synthesising it from scratch.

There are two other drugs that target the same enzyme as Tamiflu. But zanamivir (Relenza) must be taken by inhalation and is not widely available, while peramivir was dropped by US company Johnson & Johnson, which thought it unlikely to be profitable. BioCryst, the small Alabama firm that created peramivir, is still trying to find a new partner.

AMID ominous signs that H5N1 bird flu is acquiring the ability to spread more readily among people, many health authorities are pinning their hopes on Tamiflu, the only available antiviral drug known to block the replication of the virus. But can the drug really help stop an emerging flu pandemic?

Even if efforts to develop a vaccine are successful, it could take many months to produce the billions of doses needed in the event of a pandemic. By then it might be too late. So in the meantime, the World Health Organization is stepping up its efforts to acquire a massive stockpile of Tamiflu (oseltamivir), which it hopes will at least slow any emerging flu pandemic.

Recent cases of H5N1 in northern Vietnam have caused concern because of signs that the virus is changing. It has become less lethal and is occurring in larger clusters than past cases. Last month studies also revealed that the virus is diverging genetically.

It has been suggested that co-administration of Tamiflu® with another drug called probenecid could dramatically extend the world's limited supply of Tamiflu®. Probenecid reduces excretion of Tamiflu®'s active metabolite. 500mg of probenecid given every six hours doubles Tamiflu®'s maximum blood concentration and also doubles the time that Tamiflu® stays in the blood, multiplying a patient's overall exposure to the drug 2.5-fold. Avian Flu Treatment: Antiviral Drugs

Four antiviral drugs (amantadine, rimantadine, zanamavir and oseltamivir) have been approved by the U.S. Food and Drug Administration (FDA) for treatment of influenza. These need to be taken within 2 days after becoming sick. If taken at the appropriate time, these drugs can reduce avian influenza symptoms and may shorten the time you are sick by 1 or 2 days and also make you less contagious. Two drugs (in the neuraminidase inhibitors class), oseltamivir (commercially known as Tamiflu) and zanamivir (commercially known as Relenza) can reduce the severity and duration of illness caused by seasonal influenza. At present manufacturing capacity, which has recently quadrupled, it will take a decade to produce enough oseltamivir to treat 20% of the world's population. The manufacturing process for oseltamivir is complex and time-consuming, and is not easily transferred to other facilities.

So far, most fatal pneumonia seen in cases of H5N1 infection has resulted from the effects of the virus, and cannot be treated with antibiotics. Nonetheless, since influenza is often complicated by secondary bacterial infection of the lungs, antibiotics could be life-saving in the case of late-onset pneumonia. Customs agents have intercepted more than 50 shipments of counterfeit Tamiflu, the antiviral drug being stockpiled in anticipation of a bird flu pandemic, marking the first such seizures in the U.S., authorities said Sunday.

The first package was intercepted Nov. 26 at an air mail facility near San Francisco International Airport, said Roxanne Hercules, a spokeswoman for U.S. Customs and Border Protection.

Since then, agents have seized 51 separate packages, each containing up to 50 counterfeit capsules labeled generic Tamiflu.

The fake drugs had none of Tamiflu's active ingredients, and officials were running tests to determine what the capsules did contain. Initial tests indicated some vitamin C in the capsules, said David Elder, director of the Food and Drug Administration Office of Enforcement.

Information on the packages was written in Chinese, but it is unclear where the drugs originated, Elder said.

They were sent by Asian suppliers to individuals who placed orders over the Internet, Hercules said. She said none of the shipments intercepted so far was bound for doctors or hospitals.

Agents became suspicious because Tamiflu is produced by Swiss pharmaceutical manufacturer Roche, and there is no generic version available.

"What we're trying to do is alert the American public that they shouldn't be buying this product because we may never be able to track down the manufacturers," Elder said Sunday. "We've anticipated the likelihood of counterfeits from the very beginning. People are trying to profit on the heightened concerns of the American public."

The H5N1 strain of the bird flu virus has ravaged poultry stocks in Asia and killed at least 71 people since 2003. Tamiflu is one of two drugs found to be effective against it.

Fears of a bird flu pandemic have increased as the virus has spread into poultry in Europe. While bird flu has not yet appeared in the United States, or been found to transmit from person to person, officials worry it could mutate and, because humans have no immunity to it, become contagious globally.
